Answer TB  or XGP of the Kidney and Chronic Pyelonephritis of the Right Kidney

This series of CT scans through the kidneys show decreased function of the left kidney with hydronephrotic calyces and calcification in staghorn distribution. (a,b,c) There is an associated left psoas abscess. These findings are consistent with the known diagnosis of tuberculosis of the kidney with a secondary abscess in the psoas. The natural evolution of the disease on the left is autonephrectomy from progressive infundibular narrowing and end stage hydronephrosis. The right kidney shows naked calyces in image c, reminiscent of chronic pyelonephritis.

TB of the Left Kidney
Clubbed hydronephrotic calyces. infundibular narrowing, evolving staghorn calyx that stops short of the calyx, psoas abscess,
Differential diagnosis is Xanthogranulomatous Pyelonephritis (XGP)
